We are now registered to see patients under the NDIS (National Disability Insurance Scheme).

What does this mean for GP’s?

While patients under NDIS don’t require a referral from a GP they do need to be diagnosed as having a permanent disability.

All services under the NDIS are totally funded, with no out of pocket expenses for your patients.

Permanent disabilities can be in the realm of

– developmental
– physical
– mental health

Revive can see people for:

1. Therapy – Psychologists

Therapy to help people reach their goals of engaging socially, recreationally, and occupationally.

Carla Bormolini, Psychologist is taking a majority of referrals for NDIS.

The rest of the team are also taking NDIS referrals depending on the nature of the referral.

2.  Health Improvement – healthy diet, physical training and psychology 

Misty Reinkowsky, Dietitian
NDIS also funds your patients to receive assistance with eating healthily and organising their dietary plans.

Diet can be complemented with an individualised exercise programme.

Marc Bormolini, Personal Trainer

Exercise can be a land based programme involving walking and stretching exercises and providing accountability to build strength or lose weight.  Or it may be based at a gym if the participant prefers and has the funding for such.

3.  Employment counselling 

Linda Bracken, Employment Consultant has joined our team to provide:

– career counselling
– career planning
– assessment tools to assist with decision making
– locating work experiences
– job seeking skills including resume writing and practicing interview skills

Where needed she can liaise with other agencies for support workers to provide on the job support

4.  Therapy assistant

To assist with exposure therapy goals.

What does your patient need to do?

To be placed on an NDIS plan they may require a letter from yourself to confirm that their disability is permanent.

Step one: See a Local Area Coordinator for assessment and provide a NDIS plan

Step two:  A support coordinator may be allocated to help implement the plan

Step three: Refer the patient to Revive for any of our services.

They may benefit from speaking to professionals prior to their meeting to understand how many hours may be needed for which services
