This scale helps clinicians get an indication of your levels of depression, anxiety and stress over the last week. Don’t be alarmed if you find yourself with a “severe” indication in any of the columns, but please do make contact with us so we can help bring you back to better health and happiness.

Please read each statement below (or download the PDF version here) and select an option that indicates how much that statement applied to you over the past week. There are no right or wrong answers. Don’t spend too much time on any one statement.

The rating scale is as follows:

(0)- Did not apply to me at all – NEVER
(1)- Applied to me to some degree, or some of the time – SOMETIMES
(2)- Applied to me to a considerable degree, or a good part of time – OFTEN
(3)- Applied to me very much, or most of the time – ALMOST ALWAYS
